Get Ready for Your First Visit to CPS

Below, we have provided links to our Patient Forms for adults and children.
It would be very helpful if you would complete the appropriate forms before your first appointment and submit them online to us. If you can’t submit them online, please bring the completed forms with you to your first appointment.

Please note, the left column of the forms below is for children/youth and the right column is for adults. The middle column contains “General Forms.” Please review, complete, digitally sign (if needed on that form), and download these “General Forms” and submit them along with your completed Intake Form and Outcome Questionnaire.

How To Upload Your Digital Files

Click on the appropriate Intake Form below, complete the form, digitally sign it, and download it to your computer.
Follow the same procedure for the Outcome Questionnaire and each of the “General Forms,” downloading each to your computer.
(Please note that the only “General Forms” you need to send us are the ones that require a signature. The others are for your information and do not need to be sent back to us.)
Once completed, click the blue icon on the right side of your screen that says “Send us a file.”
On the next screen click the paper clip icon next to the box that says “Select Your Files.” This will pull up your downloads. Attach all of your completed forms. They will load in the “Select Your Files” box.
Complete all of the other boxes on that page and click “Send” at the bottom. That’s it!
